CONFIDENTIALITY AGREEMENT Information shared in spiritual mentor sessions is confidential and will not be discussed or released to anyone, except in cases where there is sufficient cause to believe that a life is in danger, whether your own or another’s. In addition, in an attempt to gain perspectives and ideas as to how best to help you reach your goals, I may meet with other professionals regarding supervision for our sessions, but will not identify specific information being disclosed. Spiritual mentors are also required by law to report child abuse, child sexual abuse, elder abuse or intentions to harm self or others. If you have any questions or reservations about the policy in regard to confidentiality, then the policy should be discussed before signing below. By signing below you are accepting the confidentiality policy, its limits and exceptions.
SPIRITUAL MENTOR AGREEMENT Spiritual mentorship is not a form of or substitute for mental health therapy or professional counseling. It can act as a supplement if you are already in counseling. If you are currently in therapy, it is your responsibility (at your own discretion) to alert your therapist that you are also seeing a spiritual mentor. As holistic and integrated beings, it may be appropriate at times for us to discuss psychological and relational matters. It is important to recognize though that I am not a psychotherapist nor do I provide those services. If I recognize matters that we are discussing are outside my professional field, I may redirect you to a therapist. In spiritual mentorship, our main focus pertains to and concerns your spiritual life. As a spiritual mentor, it is not my role to tell you what to believe or what to do. I am here to provide practical tools and methods for deeper connection with your spiritual path. Ultimately, you are responsible for your own spiritual life and what tools you utilize. My standard fee for one session is $100, payable at the time of booking. I do also provide package options and a sliding scale option for low income clients. Inform me if you would like to utilize either of these options. Each session will last 55 minutes, beginning at the mutually agreed upon time. Notification should be given within twenty-four (24) hours if an appointment cannot be kept by either party. Without such notification, there will be no refund of the fee. If payment for past or missed sessions have not been made, I reserve the right to deny services until the payment is received.
